2- Acknowledgment and Agreement: You have read, understand, and agree to be legally bound by the Terms of Service and the Privacy Policy in their entirety.
3- Authority to Agree: You represent and warrant that you are authorized, either in your individual capacity or as an authorized representative of a company, organization, or other legal entity, to accept the Terms of Service, provide information pursuant thereto, and bind such individual or entity to these Terms.
4- Indemnification for Misrepresentation: You agree to indemnify, defend, and hold harmless FMCSA Processing Agent LLC from and against any claims, losses, liabilities, damages, costs, or expenses arising out of or related to any false, inaccurate, or unauthorized representations made by you.
5- Formation of Binding Agreement: By submitting a payment through this website, a legally binding agreement is formed between you and FMCSA Processing Agent LLC (#1 A $20 BOC-3 Filing LLC), governed by and subject to the Terms of Service and related policies.

By submitting your payment information, you further represent, warrant, and certify the following:
1- Accuracy of Information Under penalty of perjury pursuant to the laws of the United States of America, you certify that all information provided by you in this form is true, accurate, and complete.
2- Authority to Act and IndemnificationYou represent and warrant that you are duly authorized, either in your individual capacity or as an authorized representative of a company, organization, or other legal entity, to submit the credit card information provided and to incur charges for this service. You agree to fully indemnify, defend, and hold harmless FMCSA Processing Agent LLC from and against any claims, disputes, losses, liabilities, or damages arising from any false, inaccurate, or unauthorized representations made by you.
3-Subscription Authorization and Automatic RenewalYou acknowledge and understand that the BOC-3 service is provided on a subscription basis and is subject to an automatic annual service fee of $20.00, which will be charged every twelve (12) months to the credit card you have provided. By submitting your payment information, you expressly authorize FMCSA Processing Agent LLC to initiate all current and future automatic charges associated with the BOC-3 service to the credit card on file until such time as the service is properly canceled in accordance with our written cancellation requirements.
4-Final Sale and No-Refund PolicyYou acknowledge and agree that all charges related to the BOC-3 filing and associated services are final. No refunds, credits, or reversals—whether partial or full—will be issued once your credit card has been charged, regardless of usage, duration, or subsequent cancellation.
